Our staff is friendly and always ready to help with scheduling, insurance or anything else you may need during your time with us. Our doctors will take extra care to listen to your concerns, answer all your questions and thoroughly explain your condition and treatment options.
Appointments:
You are welcome to call us to see if there are any changes as each of our doctors has a schedule tailored for your convenience and may change from time to time. To schedule an appointment, please call us at 903.533.8811 or use our online Request an Appointment form. If you are unable to keep your appointment, please notify us at least 2 business days in advance. Since our doctors want to spend as much time as needed with each patient, any patient arriving late by 15 minutes or more for an appointment will be asked to reschedule. Being punctual helps the doctors stay on schedule and therefore keeps each patient's wait time as short as possible.
New Patient Forms:
Once you have made your initial appointment, please click on the links below to download our New Patient Forms. Please print and complete each form prior to your arrival to help speed up the new patient registration process. Please arrive at least 15 minutes prior to your appointment so that we can process your paperwork. All new patients must present a valid drivers license at time of check-in. Do not forget to bring a valid driver's license, insurance card and a list of all medications.

Prescriptions:
Please contact your pharmacy at least ONE WEEK PRIOR to the end of your medication so that a prescription renewal request can be faxed to our office. This will ensure all prescriptions are filled in a timely manner.
Insurance:
We are accepting new patients and we accept most insurance plans.
Four Seasons Women's Health does not voluntarily accept Medicaid for OB/Gyn services. If you would like to be seen and not file on Medicaid, please click here.
